I don't know your situation, but if he left you and there wasn't a good reason I would let him go. If not, then it's worth a try.

You can go on from there. Just say what is in your heart, but don't corner him. Leave the decision up to him.

I'd consider doing this in person, not in a letter. Ask him out for coffee, or create an excuse to get together. After an hour of conversation, you will have a pretty good idea if you want to see each other again.
